Additional Forms Needed

In order to complete the application process, the school will need the following:

1. Recent report card and/or transcript
2. Past school records
3. Teacher Recommendation Form(s):
   Please use the Teacher Recommendation Form(s) for the grade level your child is applying for:
   1. PK3 - Kindergarten
   2. First Grade - Fifth Grade (Note: Fifth Grade applicants need to submit two teacher recommendation forms - one for math and one for language arts)
   3. Sixth - Eighth Grade Math
   4. Sixth - Eighth Grade Language Arts
4. Standardized test scores (Stanford, OLSAT, ITBS, ISEE, ERB CTP, CPAA)
5. Birth Certificate
6. Divorce Decree (if applicable)
