Important Summer Information and Forms
Important dates for the 2019-2020 school year:
- Thursday, August 1: All summer forms, including updated immunization record (or exemption), due to the office
- Thursday, August 15: First day of school; half-day with times that vary by wing
Required Immunizations must be administered and immunization record (or exemption) on file prior to attending class.
An independent screener will be available at the school in October to perform state-mandated vision and hearing screening, Acanthosis Nigricans (AN) screening, and spinal screening for a nominal fee. You are welcome to have your child’s physician perform these screenings over the summer, if you prefer. The screening requirements are as follows:
- Vision and hearing screening: required for 4-year-olds, K, 1st, 3rd, 5th, 7th, and new students
- Acanthosis Nigricans (AN) screening: required for 1st, 3rd, 5th, and 7th grade
- Spinal screening: required for 5th and 7th grade girls and 8th grade boys
If you are having your child's physician perform these screenings rather than using the onsite screener in October, the results (or exemption) are due to the office by August 1. The physician may record the results of the AN and spinal screening on the health form.
All NEW students, students with a previous medical condition, and students under the age of 4 must turn in a Health Form by August 1, 2019.
Vision/Hearing Screening Requirements and Vision/Hearing Screening Form
(Any new students, 4 year olds, Kindergarten, 1st, 3rd, 5th, and 7th)
Texas Department of State Health Services clearly states the Vision/Hearing Screening Requirements and provides the Vision/Hearing Screening Forms (M-40 and M-60).
All students must submit current immunization records or appropriate exemption form. Immunization requirements are provided by the Texas Department of State Health Services.
Internet Use Agreement
All students need an Internet Use Agreement form on file with the office to be permitted to use computers at school. Please select the appropriate form below for the grade level your child will be going into next school year.

Summer Reading is incredibly important and can be lots of fun too! Below are the summer fun reading lists (PK3-K) and required reading lists (1st-8th) listed by grade level for all our TBS students. Please note that you should use the list for the grade level your child will be going into this coming school year. Happy Reading!
