Important Summer Information and Forms
Important dates for the 2018-2019 school year:
- Tuesday, July 31: All summer forms, including updated immunization record (or exemption), due to the office
- Thursday, August 16: First day of school
Required Immunizations must be administered and immunization record (or exemption) on file prior to attending class.
An independent screener will be available at the school in October to perform state-mandated vision and hearing screening, Acanthosis Nigricans (AN) screening, and spinal screening for a nominal fee. You are welcome to have your child’s physician perform these screenings over the summer, if you prefer. The screening requirements are as follows:
- Vision and hearing screening: required for 4-year-olds, K, 1st, 3rd, 5th, 7th, and new students
- Acanthosis Nigricans (AN) screening: required for 1st, 3rd, 5th, and 7th grade
- Spinal screening: required for 6th grade
If you are having your child's physician perform these screenings rather than using the onsite screener in October, the results (or exemption) are due to the office by July 31. The physician may record the results of the AN and spinal screening on the health form.
All NEW students, students with a previous medical condition, and students under the age of 4 must turn in a Health Form by July 31, 2018.
